PACE KC helps older adults in Jackson County, MO live safely and well in their own homes.  We know that the best place for most of us to live as we age is at home, not in a nursing home. So, we provide all-inclusive care and services for seniors to help them live in the homes and communities they love for as long as possible.

Cost is not a barrier

If you are enrolled in MO HealthNet (Medicaid) you will pay nothing or you will pay your MO HealthNet "spend down." When you apply to PACE KC, we can help you apply for Medicare and MO HealthNet if you don't already have coverage.

No extra expenses

PACE KC covers all the care our participants need with no deductibles, co-pays, or out-of-pocket expenses. If a service is covered by Medicare or Medicaid, provided by a PACE KC network provider, and deemed necessary by our interdisciplinary care team, we will pay for it. You may be liable for any out-of-network costs if they do not receive prior approval from our Interdisciplinary Team.

You are in control

You can disenroll from PACE KC at any time for any reason. To disenroll, just notify PACE KC of your intent to disenroll. You will be asked to sign a disenrollment form. Disenrollment takes effect the first day of the following month.

We help you live where you feel at home

PACE KC is a healthcare plan designed exclusively for seniors. If you are age 55 and older, dealing with multiple health challenges and need assistance in your daily routine, PACE KC might be just the help you need.

We personalize your care

We help everyone differently. You and your care team will work together to create a personalized care plan that may include some or all of PACE KC's services. These depend on your medical needs, which may change over time. Care is provided at our adult wellness center or in your home.

We get you out of the house and having fun

Having a social life and friends is a great way to support your overall health. PACE KC offers both structured and unstructured social opportunities through art, music, events, exercise, and field trips.
