As a valued PACE KC participant, you will have access to primary care, rehabilitation therapy, and other routine medical services at the PACE KC Adult Wellness Center. Additionally, you can participate in PACE KC classes, outings, fitness sessions, meals, and recreational activities that promote socialization and an active lifestyle. Rehabilitation Therapy

At PACE KC, we strongly believe in the transformative power of physical, occupational, and recreational therapy as essential components of health for aging adults. Working closely with you and the rest of your care team, our rehabilitation therapists and life enrichment specialists will create and manage a personalized care plan tailored to your unique needs. Physical Therapy: In addition to aiding in injury recovery and post-surgery rehabilitation, physical therapy can alleviate concerns such as arthritis pain, balance and gait issues, incontinence, stroke recovery, and symptoms related to Parkinson’s, Alzheimer’s, and dementia.

Door-through-Door Transportation

To ensure your safety as you travel to and from all medical visits and PACE KC-related activities, we provide transportation that goes far beyond curbside pick-up. Our “door-through-door” transportation specialists will personally escort you through the entryway of your home and your destinations. Our drivers will escort you between your home and destinations, accommodating any needs, such as wheelchairs, walkers, or limited mobility. Experience PACE KC’s exceptional care even on the go.

Medications

PACE KC covers prescription drugs and prescribed over-the-counter medications at minimal or no cost to you. Receive your medications, including refills, during visits to the PACE KC Adult Wellness Center or via our convenient Meds2Home pharmacy program. With Meds2Home, enjoy easy-to-follow daily regimens in color-coded, smart-labeled boxes containing pre-sorted pouches and clear dosage instructions. Social Work

PACE KC social workers connect you to vital community resources, including food, financial, and housing assistance. Offering counseling and referral services when needed, they ensure continuous care by helping maintain your financial eligibility for PACE KC. At the PACE KC Adult Wellness Center, we offer nutritious meals and snacks alongside comprehensive nutrition services. Home Care Services

PACE KC offers comprehensive home care services to eligible participants, as determined by our Home Care Coordinators and the interdisciplinary team. These services include personal care, housekeeping, shopping, meal preparation, and medical reminders. Our nurses also provide in-home support, including blood pressure checks, medication compliance, blood draws, and wound care as needed.

Urgent Care Services

In times of urgent healthcare needs that don’t require emergency services, you can count on PACE KC for expert assistance. Our knowledgeable and caring team is available 24/7 to provide timely guidance for issues such as fevers, abdominal pain, nausea, vomiting, or difficulty urinating. No matter where you are, you can trust us to authorize coverage for urgent care, even outside our service area.

Nursing Home Care

At PACE KC, our primary objective is to enable you to stay at home and in the community as long as possible. However, in some cases, your care team may recommend a short-term or long-term nursing home stay for your care and safety. Rest assured, our experts will involve you and your loved ones in this decision-making process, provide the necessary coverage, and manage the entire process for you.

Emergency Services

As a PACE KC participant, you have access to emergency services anywhere within the United States or its territories. Prior authorization is not required for emergency medical treatment. In the event of a medical emergency, call 911 or seek treatment at the nearest emergency room immediately. Experience compassionate care when you need it most with PACE KC.

End-of-Life Care

As you age and your health conditions change, the focus of care may shift from treatment-focused to comfort-focused. At PACE KC, we understand this transition and provide expert End-of-Life Care to support and manage it. Our team offers pain control and other treatments to deliver comfort and peace of mind during your final days. PACE KC is responsible for 100 percent of your medical services, as long as:

The service is included in your Plan of Care
The service is received from an in network provider

The service is medically necessary
The service is approved by your PACE KC Team

Coverage & Non-Discrimination Statement

PACE KC provides all the care and services covered by Medicare and Medicaid (as authorized by your interdisciplinary team).

PACE KC participants must receive all necessary healthcare, including primary care and specialty physician services (other than emergency services), from PACE KC network providers or other providers authorized by PACE KC. PACE KC participants may be fully and personally liable for the costs of unauthorized services or out-of-network services.

PACE KC will not discriminate on the basis of race, ethnicity, national origin, religion, sex, age, sexual orientation, mental or physical disability, or source of payment.
